What Are Scheduled Home Visits?

Medicare defines four levels of hospice care. Scheduled home visits — sometimes called routine home care — is the level most patients receive most of the time.

It means a care team comes to you on a planned schedule, in the place your loved one calls home. No hospital admissions, no waiting rooms, no travel on difficult days.

Who Visits, and How Often?

Hospice nurses assess symptoms, manage medications and adjust the care plan. Visit frequency depends on how the patient is doing — often once or twice weekly at first, increasing as needs change.

Home health aides assist with bathing, dressing, grooming and personal care, typically several times a week.

Social workers help with practical matters, advance planning and the emotional weight families carry.

Chaplains offer spiritual support of whatever kind fits your family, including none at all.

Volunteers provide companionship and give caregivers a short break.

Does the Visit Schedule Change Over Time?

A care plan written at admission is not fixed. As a patient’s condition shifts, visit frequency increases, and the mix of who comes changes with it.

Families often worry that hospice means fewer visits over time. The opposite is true. Presence increases as needs grow, and in the final days visits become daily or more.

Scheduled visits are only part of it. Our clinical line is answered live 24 hours a day, every day of the year, including weekends and holidays.

If something changes at 2am — pain that will not settle, breathing that frightens you, a fall — you call and reach a person who knows your case and can guide you or dispatch a nurse. If symptoms escalate beyond what routine visits can manage, we move to continuous care.

Home means wherever your loved one lives — a house, an apartment, an assisted living community or a long-term care facility. We come to all of them.

What Happens on a Hospice Nurse Visit?

A hospice nurse checks vital signs, assesses pain and other symptoms, reviews how medications are working, examines skin and mobility, and updates the care plan.

Just as importantly, they answer questions. Families rarely know what to expect next, and a good nurse explains what is happening and what is likely to come — which removes a great deal of fear.
