What Does Continuous Home Care Involve?

A nurse remains in the home for an extended period — Medicare requires a minimum of eight hours in a 24-hour period, predominantly nursing care — working continuously to bring symptoms under control.

That means frequent medication adjustment, close monitoring, hands-on intervention, and direct communication with the attending physician as the situation evolves.

It also means someone stays. Families describe that presence as the difference between an unbearable night and a manageable one.

How Long Does Continuous Care Last?

Continuous care is not a permanent level. It is provided during a crisis and continues until symptoms stabilize — usually a matter of hours or days.

Once the patient is comfortable again, care steps back down to routine scheduled home visits. If symptoms cannot be managed at home despite continuous care, inpatient care may be appropriate instead. Where the issue is caregiver exhaustion rather than symptoms, respite care is the right level.
