Medical Social Workers, Spiritual Counselors, Chaplains and other religiously-affiliated individuals may provide support for hospice clients and family members. The services are extended to family members as well, knowing that patient’s condition also affect not only the physical but their psychosocial status as well. Ameri Hospice recognizes the need to support family members during times of crises.
Services provided by Ameri Hospice through our Licensed Medical Social Workers, Chaplains, and Counselors:
- Hospital discharge and transfer to hospice care in behalf of client / family
- Coordinate transfer to Inpatient care and follow up visit
- Coordinate transfer to Nursing Facility and follow up visit
- Coordinate Respite Care
- Assisting in the development of the Care Plan
- Assist to avail community resources and financial aid
- Guidance on Healthcare Policies
- Advance Directives
- Psychosocial assessment of patient and family members
- Coordinate DME requirement
- Patient counseling
- Family Counseling either individual or in group
- Greif / Bereavement Counseling
- Helping the family adjust to loss or change
