We are dedicated to serve our patients even during the most difficult times or crisis. We will arrange and coordinate the transfer of patient to a Nursing Facility if the need for 24 hours nursing care or supervision arises. This can be a situation when nursing supervision is required for pain or symptom management. Although the patient is transferred to a Nursing Facility, Ameri Hospice will still coordinate patient’s medical condition with the family members, primary physician and the hospice Interdisciplinary Team (IDT).
Generally, Continuous Care is arranged on as needed basis for symptom management and intervention.
